INFORMED CONSENT FORM

 

INTRODUCTION

The title of this research study is, “A Qualitative Study: African American Men and Resilience to Overcome the Obstacles of Homelessness”. 

I am George Spencer Allen a doctoral student under the supervision of Dr. Kristine Quade in the College of Doctoral Studies at Grand Canyon University. The purpose of this study is to explore how formerly homeless African American men tell their story using elements of resilience such as, confidence, communication, and creativity to overcome homelessness.

KEY INFORMATION

This document defines the terms and conditions for consenting to participate in this research study.

  • How do I know if I can be in this study?

    • You can participate in this study if you:

      • Are an African American male between 21-65 years old.

      • Was homeless in Los Angeles County minimum 3 months.

      • Utilized the 3 strategies of resilience to overcome homelessness.

 

  • You cannot participate in this study if you:

    • Are not an African American male between 21-65 years old.

    • Was not homeless in Los Angeles County minimum 3 months.

    • Did not utilized the 3 strategies of resilience to overcome homelessness. 

  • What am I being asked to do? If you agree to be in this study, you will be asked to:

    • What? Complete an online 2-minute Connor-Davidson Resilience Scale (CD-RISC) questionnaire. Participant in a 1 on 1 interview approximately 60-90 minutes.

    • When? If you elect to complete the (CD-RISC) questionnaire it can be done on your availability. Participants will be chosen on a first come first serve.  If you elect to participate in a 1 on 1 interview a meeting will be scheduled based on your availability.

    • Where? Questionnaire will be on Online. The individual interview will be held via internet Zoom conferencing.

    • How? The questionnaire will be completed via SurveyMonkey. The research data will be collected via internet Zoom conferencing.  

Audiotaping: I would like to use an audio/video recorder to record your responses.  You cannot participate if you do not wish to be recorded. Each interview will be audio/video recorded. The face-to-face meeting and a secondary recording device for backup will be used to audiotape the interview. The voice/video recorder(s) will record your responses which later will be transcribed for analysis. Your identity will be protected using a de-identification process and stored on a password protected computer and USB drive.

                                                                                               

Videotaping: I would like to use a video camera to record your actions.  Because this tape will show who you are, these extra steps will be taken transcribed to identify nuances found within facial expressions, reviewed by research committee, and destroyed immediately following written record. You can still participate if you do not wish to be recorded. The video will not be used to record then. Your identity will be protected using a de-identification process and stored on a password protected computer and USB drive.

 

  • Who will have access to my information?

My dissertation chair, committee members, and all College of Doctoral Studies’ reviewers may view your information and your answers.

 

Participation is voluntary. However, you can leave the study at any time, even if you have not finished, without any penalty or loss of benefits to which you are otherwise entitled. If you decide to stop participation, you may do so by stating that you would like to stop the interview. If so, I will not use the information I gathered from you.

  • Any possible risks or discomforts? There are no known risks from taking part in this study, but in any research, there is some possibility that you may be subject to risks or discomforts that have not yet been identified. 

  • Any direct benefits for me? No.

  • Any paid compensation or incentives  for my time?

There is no paid compensation for your time as participants. However, to encourage participation in a one-on-one interview, those who agree to participate and complete an interview will receive a completed published copy of the dissertation.

  • How will my information and/or identity be protected?  Participant confidentiality will be assured by using assigned numbers and/or aliases and excluding any personally identifiable information. A list of names and codes will be kept on a separate USB drive in a separate location, to ensure no one will connect the data to the specific participant's names. A file with all de-identified raw data and coded data will be housed on a password-protected online storage site accessible to all committee members and the academic quality reviewer(s). All individual participant's data will be protected and kept anonymous to protect the identity of the participant. Data will be destroyed ten years after the publication of the dissertation as mandated by Grand Canyon University. Paper documents will be shredded, and all electronically stored data will be bleached.

  • Will researchers be able to link participant data back to them? Researchers will not be able to link participant data back to them. This study will use de-identification procedures that will not allow researchers to link data back to participants.

  • How will the data be secured and where? A list of names and codes will be kept on a separate USB drive in a separate location, to ensure no one will connect the data to the specific participant's names. A file with all de-identified raw data and coded data will be housed on a password-protected online storage site accessible to all committee members and the academic quality reviewer(s).

  • How long will the data be kept and secured?  Minimum of 10 years.

  • Who will have access to the secured data? Only the researcher will have access to the secured data.

PRESENTATION OF INFORMATION COLLECTED

The data will be presented as a published dissertation. There are no intentions for this current work to be presented at a conference. The data will be grouped to gain a collective understanding of resilience and how it is used by African American men in overcoming adversities. 

PRIVACY AND DATA SECURITY

  • Will researchers ever be able to link my data/responses back to me?  No.

  • Will my data include information that can identify me (names, addresses, etc.)? No. Participant confidentiality will be assured by using assigned numbers and/or aliases and excluding any personally identifiable information.

  • Will researchers assign my data/responses a research ID code to use instead of my name?   Yes.

    • If yes, will researchers create a list to link names with their research ID codes?  Yes. Participant confidentiality will be assured by using assigned numbers and/or aliases and excluding any personally identifiable information.

    • If yes, how will researchers secure the link of names and research ID codes? How long will the link be kept? Who has access? Approximate destroy date?  

To secure the link of names and research ID codes protective measures (see next bullet) will be fully practiced for ten years, accessible only to the researcher and research committee and will be approximately destroyed 4/30/2033.

  • How will my data be protected (electronic and hardcopy)? A file with all de-identified raw data and coded data will be housed on a password-protected online storage site accessible to all committee members and the academic quality reviewer(s). All individual participant's data will be protected and kept anonymous to protect the identity of the participant.

  • List the data that will be protected  video files, informed consents, interview transcripts.

  • Where will the data be protected? a password-protected online storage site accessible to all committee members and the academic quality reviewer(s).

  • How long will the data be kept in the protected space?  Minimum is 10 years

  • Who will have access to the protected data? The researcher.

FUTURE RESEARCH

Once identifiers (name, address, etc.) are removed from these data collected for this study, the information could be used for future research studies or distributed to other investigators for future research studies without additional informed consent from you.
